Blue Cross / Blue Shield
  Coverage for office visits, hospitalizations, surgery, lab, x-rays, prescriptions, emergency room services, mental health and substance abuse treatment, vision. AFS pays 80% of the cost of this benefit.
Delta Dental
 

Coverage for preventative, basic/routine, restorative, and orthodontic services. AFS pays 25% of the cost of this benefit.

Flexible Benefits Plan
 

Contributions towards medical and dental premiums are pre-tax. Flexible Spending Account allows for the setting aside of a portion of pay for unreimbursed health care and/or dependent care expenses. FSA contributions are not subject to income taxes.

Long-Term Disability
 

Coverage for 66.67% of gross earnings after 90 days of total disability. AFS pays 100% of the cost of this benefit.

Group Term Life Insurance
 

Coverage for 1x annual earnings. Dependent life is provided for spouse and/or children. AFS pays 100% of the cost of this benefit.

Optional Life Insurance
  Guaranteed issue for up to two times annual salary at low group rates. Additional amounts available with application. Premiums paid via payroll deduction.
Employee Assistance
  RIEAS provides confidential assessment and counseling in times of need. Also provides referrals for medical, psychological, legal, and financial issues. AFS pays 100% of the cost of this benefit.
Worklife Solutions
  ResourcesOnCall Outside service provides assistance in locating resources such as childcare providers, adoption services, colleges, elder care resources, contractors, and more. AFS pays 100% of the cost of this beneift.
Adoption Assistance
  AFS will reimburse up to $2,500 towards the cost of adoption of a dependent child.
Paid Time Off
  Earn paid time off at a rate equal to 3, 4, 5, 6, or 7 weeks per year, depending on your length of service. This flexible time off may be taken for vacation, personal reasons, or illness.
Paid Holidays
  New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.
