MEDICAL
INSURANCE & VISION COVERAGE
For 2009, AFS is happy to announce that we are
remaining with Blue Cross & Blue Shield of RI. While we
will incur increases in premiums, we are very satisfied that
we were able to negotiate the lowest possible rates for the
best coverage available. Our group plan offers comprehensive
coverage for office visits, hospitalization, surgery, lab,
x-rays, prescriptions, emergency room services, mental health
and substance abuse treatment.

| Blue
Cross provides coverage for one routine eye exam per
calendar year. AFS will also reimburse each covered member
up to $100 per year for eye glasses or contact lenses
(reimbursement is through Primarily Care). |

DENTAL
INSURANCE
Delta Dental provides for preventative care, basic procedures,
and major procedures with a maximum calendar year benefit of
$2,000 per covered individual. Delta Dental also offers coverage
for orthodontic expenses for dependent children with a lifetime
maximum benefit of $1,000.

SECTION
125 FLEXIBLE BENEFITS PLAN
All payroll deducted employee contributions toward medical
and dental insurance premiums will be paid with pre-tax dollars.
You will benefit from a higher take-home pay.

FLEXIBLE
SPENDING ACCOUNTS
This benefit allows for setting aside a portion of pay for
un-reimbursed health care, dependent care, and/or transportation
expenses. FSA contributions are not subject to income taxes.
The maximum amount you can contribute to a medial FSA is $3,000
per year. The maximum you can contribute to a dependent care
FSA is $5,000 per year.

GROUP
TERM LIFE
This policy, with Lincoln Financial, provides group term life
insurance and accidental death and dismemberment insurance
each in an amount equal to your gross annual earnings (salary
plus commission, if applicable). Dependent coverage is also
provided in the amount of $5,000 for a spouse and $2,000 for
each child. AFS pays 100% of the cost of this coverage.
LONG
TERM DISABILITY
This policy, with Lincoln Financial, will pay a benefit
equal to 66/67% of your monthly earnings after a ninety
day waiting period. AFS pays 100% of the cost of this benefit.

| VOLUNTARY
LIFE INSURANCE
Lincoln Financial will guarantee coverage for up to two
times your annual salary at low group rates. If the amount
you elect is greater than the guaranteed issue, an application
will be required. Subsequently, medical underwriting may
be required. The maximum coverage allowed is five times annual
earnings or $500,000 whichever is less. The maximum coverage
allowed for your spouse is 50% of your elected/approved coverage
amount. |

SHORT
TERM DISABILITY INSURANCE
This policy, with Lincoln Financial, will pay a benefit equal
to 60% of your basic weekly earnings (including bonuses and
commissions, where applicable). The maximum benefit is $1,500
per week. The cost of this coverage will vary depending on
your age. There is a pre-existing condition limitation clause.
This coverage will benefit those RI employees earning over
$56,500 per year and all of those employees working outside
of RI.
